About the Program
The Fourth Trimester Program is a new model of postpartum care designed to meet the complex needs of the community and bridge the care gap during the significant postnatal period from birth through the early postpartum period. This program provides care navigation services supplementing and expanding upon the traditional post-delivery care model where patients are typically not seen until six weeks after birth. The Fourth Trimester Program offers a joint postpartum-newborn clinic.
Who Qualifies?
Postpartum patients experiencing acute issues or who had complex pregnancies or deliveries and who are within 8 weeks of birth meet with an Obstetrics provider, a Pediatrician provider certified in lactation consults, and a Registered Nurse navigator in a single visit. Patients seen at the Fourth Trimester Clinic will be linked to other needed services including: Maternal Mental Health, Pelvic Medicine, Physical Therapy, Primary Care, Postpartum Doula Services, and more.
